Supplies / Test Request Forms
ViroMed provides its clients with specimen collection and transport materials. These
include swabs, serum vials, shipping cartons, and specialized media ideally suited for
transporting specimens. Also available are test request forms preprinted with the client's
name, address and account number.
To order supplies or test request forms, fax a completed copy of ViroMed's Supply Request
Form to Client Services at 952-939-4012. For more information, contact Client Services.
Reporting
Preliminary and final reports are generated based on the type of procedure performed.
Reports include both test results and interpretations.
Billing
Client Billing:Clients are billed the last day of each month for tests completed
during that time period. Statements include individual patient billings by test and date.
Payment terms are net 30 days.
Third Party Billing: ViroMed accepts assignment for lab work performed on out-patient
Medicare patients. Please include the patient's Medicare number, date of birth, address, sex,
diagnosis, diagnostic code, name of referring physician and his/her Medicare ID# on the test
request form.
